Terms of Service

1. Acceptance of Terms

Welcome to IntegrityYou PLLC ("IntegrityYou"), a telemedicine weight management clinic operated by Dr. Jane Mattei, MD (licensed in Texas and Massachusetts) and managed by Carlos Cano. By accessing and using our website, mobile application, and telehealth services, you agree to be bound by these Terms of Service in their entirety. If you do not agree to any portion of these terms, please do not use our services.

These Terms of Service are effective as of January 1, 2025, and may be updated periodically. Your continued use of our services constitutes your acceptance of any changes made to these terms.

2. Description of Services

IntegrityYou provides telemedicine weight management services designed to help patients achieve their health and wellness goals through evidence-based medical guidance and personalized treatment plans.

Our services include:

  • Virtual Medical Consultations: Initial assessments and follow-up appointments with Dr. Jane Mattei via secure video conferencing or telephone
  • Prescription Medication Management: Evaluation, prescription, and ongoing management of FDA-approved weight management medications, including GLP-1 receptor agonist medications such as semaglutide
  • Medical Guidance: Personalized recommendations regarding nutrition, physical activity, behavioral modification, and lifestyle changes
  • Pharmacy Coordination: Coordination with our partnered licensed compounding pharmacy for medication sourcing and delivery
  • Ongoing Monitoring: Regular follow-up appointments and progress monitoring to assess treatment effectiveness and safety

All medications are sourced through a licensed compounding pharmacy, which operates under strict FDA regulations and pharmaceutical standards.

3. Patient Eligibility Requirements

To use IntegrityYou services, you must meet the following eligibility criteria:

  • Be at least 18 years of age
  • Currently reside in Texas or Massachusetts
  • Possess a valid government-issued photo identification
  • Have a valid email address and reliable means of communication
  • Provide complete and accurate medical history information
  • Be able to participate in virtual consultations using compatible technology

Patients who do not meet these eligibility requirements will not be permitted to establish care with IntegrityYou. Existing patients who move out of Texas or Massachusetts should notify us immediately, as we are unable to continue providing services to patients outside these states.

4. Telehealth Consent and Acknowledgment

By using IntegrityYou's telehealth services, you acknowledge and consent to receiving medical services through electronic communication, video conferencing, and other digital means.

Nature of Telehealth: Telehealth allows you to receive medical consultation, evaluation, and treatment recommendations without an in-person physical examination. While our physician will conduct a comprehensive medical history and assessment, a physical examination cannot be performed through telehealth.

Technology Requirements

  • Stable internet connection (broadband or mobile data)
  • Device capable of video conferencing (computer, tablet, or smartphone)
  • Private and secure location for consultations to protect your privacy
  • Functional camera and microphone for video appointments

Limitations of Telehealth

  • Telehealth cannot replicate all aspects of in-person medical examination
  • Technical difficulties may interrupt or delay your appointment
  • Certain conditions may require in-person evaluation, which we will discuss with you
  • Prescription medications require careful medical evaluation; not all patients may be candidates
  • Emergency conditions require immediate in-person emergency medical services, not telehealth consultation

Patient Responsibilities in Telehealth

  • Test your technology before appointments to ensure proper functionality
  • Be in a private, secure location during consultations
  • Provide accurate and complete information during assessments
  • Notify us of any technical difficulties immediately

5. Medical Disclaimers and Important Safety Information

You acknowledge the following important medical information:

Not Emergency Services: IntegrityYou does NOT provide emergency medical services. Telehealth appointments are not appropriate for acute medical conditions, severe symptoms, or medical emergencies. If you experience a medical emergency, please call 911 or proceed to the nearest emergency room immediately.

Limitations of Telehealth Care

  • Telehealth is not a substitute for in-person medical care when physical examination is medically necessary
  • Our physician may refer you for in-person evaluation if deemed necessary for your care
  • Telehealth services cannot diagnose conditions that require physical examination, laboratory testing, or imaging
  • If you have significant medical conditions or take multiple medications, you should have an established relationship with a primary care physician

Individual Results Vary

Weight management outcomes and medication effectiveness vary significantly between individuals. Factors such as genetics, metabolism, adherence to lifestyle modifications, underlying medical conditions, and other medications can affect results. No guarantees are made regarding weight loss, health improvements, or specific outcomes. Some patients may experience minimal results despite treatment adherence.

Potential Risks and Side Effects

Weight management medications, particularly GLP-1 receptor agonists, carry potential risks and side effects. These may include nausea, vomiting, diarrhea, constipation, abdominal pain, pancreatitis, thyroid concerns, gallbladder issues, kidney function changes, and other adverse effects. You will receive detailed information about risks and side effects during your consultation. It is essential that you report any side effects or adverse reactions to our medical team immediately.

6. Prescription Medications

The prescription of weight management medications is at the sole discretion of Dr. Jane Mattei, based on a comprehensive medical evaluation, your medical history, existing conditions, current medications, and clinical judgment regarding the appropriateness and safety of treatment.

GLP-1 Receptor Agonist Medications

  • Medications such as semaglutide are FDA-approved for chronic weight management in eligible patients
  • These medications are prescribed only after thorough evaluation and assessment of your medical suitability
  • Not all patients are candidates for these medications due to medical contraindications or risk factors
  • You must disclose all medical conditions, medications, and health history accurately
  • Ongoing monitoring is required during treatment to assess safety and efficacy

Compounding Pharmacy

IntegrityYou partners with a licensed compounding pharmacy for medication preparation and sourcing. These facilities operate under Federal Drug Administration regulations and quality standards. All medications are prepared according to USP (United States Pharmacopeia) standards and must meet rigorous pharmaceutical quality requirements.

By using our services, you acknowledge that your medications may be sourced from a compounding pharmacy rather than a traditional pharmacy. While these preparations meet FDA quality standards, they may differ slightly from commercially manufactured versions.

Medication Disclosure Requirements

You must provide a complete and accurate list of all medications you currently take, including:

  • Prescription medications
  • Over-the-counter medications and supplements
  • Vitamins and herbal remedies
  • Any medications prescribed by other physicians

Failure to disclose all medications may result in dangerous drug interactions. You are responsible for ensuring our physician has complete information about your medication regimen.

Medication Storage and Safety

  • Store all medications according to package instructions
  • Keep medications out of reach of children and pets
  • Do not share medications with others
  • Notify us immediately if you have allergic reactions or adverse effects
  • Do not alter dosages without medical guidance

7. Patient Responsibilities and Obligations

Your commitment to honest communication and following medical advice is essential to your safety and the success of your treatment.

Obligation to Provide Accurate Information

  • Provide truthful and complete information about your medical history
  • Disclose all current medications, supplements, and health conditions
  • Report any changes in your health status between appointments
  • Inform us of any allergies or adverse reactions to medications
  • Be honest about your lifestyle habits and adherence to treatment recommendations

Treatment Plan Adherence

  • Follow all recommendations provided by Dr. Jane Mattei
  • Take medications exactly as prescribed
  • Maintain regular follow-up appointments
  • Implement lifestyle modifications as recommended
  • Maintain open communication regarding treatment barriers or challenges

Reporting Adverse Effects

You must immediately notify IntegrityYou of any adverse effects, concerning symptoms, or reactions to medications. Delayed reporting of serious symptoms could result in serious health consequences. In case of medical emergency, call 911 rather than contacting our clinic.

Responsible Use of Services

  • Maintain the confidentiality of your login credentials and account information
  • Do not impersonate others or provide false identification
  • Do not attempt to circumvent security measures or access unauthorized information
  • Comply with all applicable laws and regulations

8. Pricing, Payment, and Refund Policy

IntegrityYou is committed to transparent pricing and clear communication regarding costs associated with our services.

Pricing Information

  • All fees and costs will be clearly communicated before services are rendered
  • Consultation fees, medication costs, and any additional charges will be itemized
  • Pricing information is available on our website and will be confirmed during your consultation
  • Pricing may vary based on medication type, dosage, and supply quantity

Payment Methods

  • Payment must be made in full prior to service delivery, unless alternative arrangements have been made
  • We accept major credit cards and other secure payment methods
  • All payments are processed through secure, encrypted payment gateways
  • Insurance coverage varies; we are not responsible for insurance claim denials

Refund and Cancellation Policy

  • Consultation Refunds: If you are not prescribed medication following your consultation, you are not eligible for a refund of the consultation fee
  • Medication Refunds: Medications may be refunded if returned unopened within 14 days of receipt, provided they meet pharmaceutical return standards
  • Appointment Cancellations: Cancellations must be made at least 24 hours in advance. Late cancellations may incur a cancellation fee
  • Discontinued Treatment: If you choose to discontinue treatment with IntegrityYou, any unused medications may be refunded according to our refund policy
  • No Hidden Fees: All costs are disclosed upfront; there are no hidden, surprise, or undisclosed fees

9. Intellectual Property Rights

All content on the IntegrityYou website and within our digital platforms, including text, graphics, logos, images, videos, and other materials, are the intellectual property of IntegrityYou or are used with permission from their respective copyright holders. This includes:

  • Website design and layout
  • Written content and educational materials
  • Product descriptions and specifications
  • Software and applications
  • Trademarks and service marks

You are granted a limited, non-exclusive, non-transferable license to view and use content from the IntegrityYou website for personal, non-commercial purposes only. You may not reproduce, distribute, transmit, display, perform, or otherwise use any content without express written permission from IntegrityYou or the appropriate copyright holder.

10. Limitation of Liability

To the maximum extent permitted by law, IntegrityYou and its owners, operators, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use of our services, including but not limited to:

  • Loss of profits, data, or income
  • Damage to property or equipment
  • Business interruption
  • Loss of use or enjoyment
  • Any other damages arising from the use or inability to use our services

Medical Care Liability

While IntegrityYou takes all reasonable measures to provide safe and effective telemedicine services, telehealth care carries inherent risks that differ from in-person care. These risks include technical difficulties, communication errors, and limitations in physical examination. You assume these risks by choosing to utilize telehealth services. IntegrityYou shall not be liable for outcomes, adverse effects, or complications resulting from telehealth treatment, provided our physician followed appropriate medical standards of care.

Technology Limitations

IntegrityYou is not responsible for technical difficulties, internet outages, device malfunctions, or other technology-related issues that prevent you from accessing or using our services. We are not liable for data loss, privacy breaches resulting from your failure to protect login information, or other security issues arising from your actions.

11. Privacy and Data Protection

Your privacy and the security of your personal health information are extremely important to us. Our collection, use, and protection of your information is governed by our comprehensive Privacy Policy.

12. Governing Law and Jurisdiction

These Terms of Service and all matters related to your use of IntegrityYou services shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law principles.

You agree that any legal action, suit, or proceeding arising from or relating to these terms or your use of our services shall be brought exclusively in the state or federal courts located in Texas. You hereby consent to the jurisdiction and venue of these courts and waive any objections based on inconvenient forum.

Dr. Jane Mattei is licensed to practice medicine in Texas and Massachusetts. Services are provided in accordance with the laws of the state in which the patient is located at the time of service delivery.

13. Changes to Terms of Service

IntegrityYou reserves the right to modify these Terms of Service at any time. Changes will be effective upon posting to our website. We will make reasonable efforts to notify users of significant changes via email or through our website.

Your continued use of IntegrityYou services following the posting of revised Terms of Service constitutes your acceptance of and agreement to the changes. If you do not agree to any changes, you must discontinue use of our services immediately.

For significant changes affecting privacy rights or medical care, we will provide at least 30 days' notice prior to implementation whenever possible.

15. Severability

These terms supersede all prior negotiations, representations, and agreements, whether written or oral.

17. Disclaimer of Warranties

IntegrityYou services are provided on an "as-is" and "as-available" basis. We make no warranties, express or implied, regarding the services, including warranties of merchantability, fitness for a particular purpose, non-infringement, or accuracy. We do not guarantee that our services will be uninterrupted, timely, secure, or error-free.
